You may use this form to request the use of a space at any of the Our Lady of Divine Providence Family campuses.

Procedures

  1. Submit the form to the best of your ability.

  2. You will receive an email with one of the following decisions:

    • Approved - your request has been approved with no changes.

    • Pending - your request is pending additional follow up, such as additional paperwork, proof of insurance, etc.

    • Denied - your request has been denied.

  3. Pick up keys as directed at the Family of Parishes Office at the St. Vivian campus (7600 Winton Road, Cincinnati, OH 45224) during business hours (Monday-Friday 8:00 am-4:00 pm).

Policies

  • DEADLINES

    • Requests by parish ministries or affiliates must be made no later than two weeks in advance of the desired date.

    • Rental requests at any of our campuses must be made no later than one month in advance of the desired date. These include, but are not limited to:

      • Birthday parties

      • Baby/Bridal showers

      • Private receptions

  • FEES

    • Amounts

      • Each parish has determined the fees and/or security deposit amounts for their location. We will be publishing our fee amounts soon.

    • Payments

      • Payment is accepted via cash or check (made out to the parish from which you are renting) at our Family of Parishes Office at the St. Vivian Campus. (7600 Winton Rd, Cincinnati, OH 45224).

      • You may mail your payment or drop it off during business hours (Monday-Friday 8:00 am-4:00 pm). You can also drop your payment in the Sunday collection basket in a clearly marked envelope.

  • INSURANCE REQUIREMENTS

    • Liability Insurance

      • All rentals must provide a Certificate of Insurance in the amount of $1,000,000.00.

      • The additional insureds must include:

        • The parish at which you are renting

        • Archbishop Robert Casey

        • Archdiocese of Cincinnati

      • Insurance can usually be obtained through your homeowners insurance provider. If you need assistance in obtaining coverage, you can apply for the Catholic Diocese TULIP Insurance.

    • Alcohol Rider

      • Events that charge admission and serve alcohol must have an “Alcohol” rider.

      • Events that charge for alcohol by-the-glass must have an “Alcohol” rider.

  • ADDITIONAL PARISH-SPECIFIC POLICIES

    • Some parishes have their own additional policies in addition to these. You will receive and agree to these policies on a case-by-case basis.

  • BAPTISMS/WEDDINGS/FUNERALS
