The Disciple Maker Index

The Disciple Maker Index is a 95-question online survey tool that supports Church ministry with objective data. The survey should take about 10-15 minutes of your time.

Your responses will be included in the listening process for Synod 2027, and will also be available to OLODP Staff to help us serve you better.

Click the button for your parish below to get started. Responses are accepted beginning October 27 and through November 24.
